GLP-1 Medication Use Shows No Apparent Impact on Icotrokinra Outcomes in Phase 3 Psoriasis Analysis

Key Takeaways

  • Pooled post hoc analyses of ICONIC phase 3 studies found generally consistent icotrokinra efficacy, safety, and pharmacokinetics regardless of GLP-1 medication use.
  • Among icotrokinra-treated patients, week 16 IGA 0/1 response rates were 65% with GLP-1 exposure and 68% without; PASI 90 rates were 45% and 54%, respectively.
  • The GLP-1–exposed efficacy subgroup included only 31 patients, limiting statistical comparisons. Exposure duration varied, and dosing details were unavailable.

Concomitant use of glucagon-like peptide-1 (GLP-1)–based medications was not associated with an apparent difference in icotrokinra efficacy, safety, or pharmacokinetics among patients with plaque psoriasis, according to pooled post hoc analyses of the phase 3 ICONIC clinical trial program presented at the 2026 Fall Clinical Dermatology Conference.

Investigators evaluated data from ICONIC-LEAD, ICONIC-ADVANCE 1, ICONIC-ADVANCE 2, and ICONIC-TOTAL to assess whether GLP-1 medication exposure affected outcomes with icotrokinra, an oral peptide targeting the IL-23 receptor.

Efficacy analyses included three studies, excluding ICONIC-TOTAL because of differences in enrollment criteria and endpoints. Participants received icotrokinra 200 mg once daily or placebo during the initial 16-week treatment period.

Psoriasis Clearance and Safety Outcomes With GLP-1 Use

Among 31 GLP-1–exposed participants in the efficacy population, 65% of icotrokinra-treated patients achieved an Investigator’s Global Assessment (IGA) score of 0 or 1 with at least a 2-grade improvement at week 16, compared with 68% of patients without GLP-1 exposure.

Psoriasis Area and Severity Index (PASI) 90 responses occurred in 45% and 54% of icotrokinra-treated patients with and without GLP-1 exposure, respectively.

Among GLP-1 users, adverse events occurred in 53% of icotrokinra-treated patients and 69% of placebo recipients through week 16. Pharmacokinetic analyses also showed similar icotrokinra exposure regardless of GLP-1 medication use.

“In these pooled post-hoc analyses, ICO efficacy, safety, and PK were generally consistent regardless of GLP-1 medication use,” the investigators concluded.
