In each issue, Practical Dermatology® magazine spotlights a new device or over-the-counter or prescription treatment that dermatologists are recommending to their patients. This month, Emil Tanghetti, MD, a dermatologist in Sacramento, CA, discusses Ortho Dermatologics’ Arazlo (tazarotene) Lotion, 0.045%, which recently got the FDA nod for the topical treatment of acne vulgaris in patients nine years of age and older. Dr. Tanghetti was the lead author of the study that led to the approval.

Tell us more about Arazlo.

Emil Tanghetti, MD: This is a new formulation of tazarotene that has a unique delivery system, which enhances delivery and also improves tolerability. The lower concentration of tazarotene and the use of humectants and occlusive agents are primarily responsible for this result. The polymeric mesh permits these agents to be separated and mixed when the mesh is in contact with the skin and dissolves. This drug is as effective as tazarotene 0.1% cream with a much better tolerability profile. As with any retinoid, irritation can occur, but the use of the polymeric mesh with the emollients does improve the tolerability profile.

Who is the best candidate for this new lotion?

Dr. Tanghetti: Generally, tazarotene is a very effective retinoid in the moderate to severe acne patient but with this new vehicle, virtually all acne patients would benefit from this product.

How do you suggest patients with acne use this product? What kind of results are they seeing?

Dr. Tanghetti: I would use this agent as I would any other topical retinoid. I would also suggest it to patients using the current tazarotene cream formulations where tolerability can be a significant problem. This product could also be used with benzoyl peroxide (BPO)/clindamycin preparations in the morning and Arazlo in the evening or vice versa. This vehicle is very spreadable and could also potentially be used on the chest and back. While this is an off-label use, other forms of tazarotene have been used quite effectively on the chest and back. It could also be used in combination with a BPO in a short-contact method for five to 10 minutes before washing in the morning or the evening. The BPO could be then rinsed off, dried and then the Arazlo applied after. I also see this product as a potential help for patients with acne who have post-inflammatory hyperpigmentation. Finally, off-label this might be a good product for photodamage. The clinical study data was compelling, and we look forward to seeing similar results in our clinic.
